THE LIFE LONG CARE PROGRAM


The Life Long Care Program provides community support services to all urban Aboriginal clients, irregardless of age, who are disabled, chronically ill, frail elderly or require acute/chronic continuum of care. The LLCP provides a holistic approach to community support services, enhancing and improving  the quality of life and living for the clients, allowing them to live independently in their community.

 

OBJECTIVES:

 

  • To facilitate an ongoing needs assessment within the Aboriginal community.
  • To provide for the development of culturally appropriate long term care services.
  • To ensure that all urban Aboriginal people are assured fair and reasonable access to all long term care services and programs.

SERVICES OFFERED:

TRANSPORTATION:

 

A support service that provides transportation to medical appointments, social and cultural activities related to LLCP.

 

A registered LLC client will recieve THREE transportation services per month:

  • two transportation sercvices for medical appointments, and
  • one transportation service for LLC activity 

CONGREGATE DINING (MEAL SERVICE):

 

This service is targeted to those clients who are either in receipt or eligible to receive other life long care community support services. This service provides clients with a nutritious meal and an activity outside of their home on a regular scheduled basis.

 

FRIENDLY VISITING:

 

A support service that provides a volunteer on a one to one basis to visit an isolated senior, physically disabled adult, or person with Alzheimer Disease or other dementias, on a regular basis.

 

SECURITY CHECKS/REASSURANCE SERVICE:

 

This service provides clients with a regular telephone call to check on their safety and well-being. These calls should not be longer than five (5) minutes in length. The number of calls made to individual clients on a weekly basis will vary, dependent on the needs of the client.

 

ABORIGINAL SUPPORT SERVICE:

 

This is a service provided by LLCP to eligible Aboriginal clients living in urban communities. The purpose of the service is to support clients with information, referral, advocacy, and access to mainstream long-term care services such as Community Care Access Centres (CCACs).
